US Market Requirement - Customer Application form link to be added to the login page.

When a customer is not yet an Alcon customer today we refer them to the MAS 1.0 login page to access the below application. This is not something we can post on myalcon.com because of issues that have happened in the past. As we have done with Adi (screen shot below) we need to have a register here button that links to this url.
